The Simulator Repair History page provides repair history database records relating to the selected Simulator. Signed Repair Affidavits are also available for viewing.

Simulator Thermometer Repair History: Step-by-Step
To obtain Simulator Thermometer Repair History records:
- Left click on "QA Simulator Search" button.
- Enter Instrument number (i.e. Gxxxx) by selecting from the list or typing it.
- Left click the "Search Simulator Certification" button.
- Left click on the hyperlink "Repair history for QA Simulator Gxxxx" to view related Repair History records.
- A hyperlink in the Work Date column indicates that a signed Simulator Repair Certification PDF file is available for viewing.
